Why get the Nail & Fungal Panel
Fungal infections of the nails, skin, and hair are extremely common — and surprisingly tricky to treat well. A thickened, discolored, or crumbling nail, a persistent itchy rash, scaling, or patchy hair loss can all be fungal, but they can also mimic non-fungal conditions like psoriasis, eczema, or trauma. And even when an infection is fungal, the specific organism matters enormously, because the three main groups — dermatophytes, yeasts, and non-dermatophyte molds — respond very differently to treatment. The standard oral antifungal (terbinafine) works well against dermatophytes but can fail against molds like Fusarium and Aspergillus or certain resistant yeasts. Since oral antifungal therapy often runs for months and may require liver monitoring, it's worth confirming both that the problem is fungal and which fungus is responsible before committing to it.
This panel uses PCR to test a single sample — a nail clipping, skin scraping, or hair sample — for 18 fungal targets spanning all three groups, identifying the exact organism rather than treating by guesswork.
Who this test is for
- Suspected fungal nail infection (onychomycosis) — thickened, discolored, brittle, or crumbling nails
- Persistent skin fungal infections — athlete's foot, ringworm, or jock itch that won't clear
- Scalp or hair involvement — scaling, patchy hair loss, or suspected tinea capitis
- Skin discoloration suggestive of tinea versicolor (caused by Malassezia)
- Nail or skin problems that haven't responded to over-the-counter antifungals
- Before starting long-course oral antifungal therapy, to confirm it's fungal and pinpoint the species first
- Recurrent fungal infections that keep returning
- Higher-risk patients — those with diabetes or weakened immune systems, in whom certain fungi can become more serious
- Cases where it's genuinely unclear whether the problem is fungal at all, versus a look-alike skin condition
What it covers
The classic dermatophytes behind most nail, skin, and hair infections — Trichophyton, Microsporum (gypseum and canis/audouinii), and Epidermophyton floccosum. A range of yeasts, including Candida albicans, glabrata, parapsilosis, krusei, and the emerging multidrug-resistant Candida auris; the Malassezia species (furfur and sympodialis) responsible for tinea versicolor; and Trichosporon (asahii and mucoides). And the non-dermatophyte molds that are increasingly recognized as causes of nail infection and are often harder to treat — Aspergillus (niger, flavus, fumigatus, terreus) and Fusarium oxysporum.
Why PCR
- It identifies the exact species. Knowing whether you're dealing with a dermatophyte, a yeast, or a non-dermatophyte mold directly determines which antifungal will actually work.
- It catches the treatment-resistant ones. Molds like Fusarium and Aspergillus and resistant yeasts like Candida auris often slip past standard therapy — identifying them upfront changes the plan.
- It's far faster than fungal culture. Traditional fungal cultures can take weeks; PCR returns results in days.
- It's more sensitive. Nail and skin cultures have notoriously high false-negative rates, frequently reporting "no growth" on genuinely infected samples. PCR detects fungal DNA directly.
- It confirms the diagnosis before you commit. Verifying the infection is fungal — and which fungus — spares patients from months of unnecessary or ineffective antifungal treatment.
